Data Privacy Notice

This registration collects your identity, birth date, Manila address, and any contact details, family number, PhilHealth number, or photo you choose to provide. The Manila Health Department will use these details to register your resident record, verify your identity and residency, issue your health card, and link health services and follow-up records to the correct person.

Your record is available to authorized personnel according to their access rights. A photo is optional. For privacy questions or requests to access or correct your information, contact the health department staff handling your registration. You may also raise a privacy concern with the National Privacy Commission.
